Direct Primary Care (Individuals) - $79/month
One time enrollment fee $100
Includes:

- Referral for diagnostic testing (add'l fees apply)
- Collaboration with specialist if needed
- Discounts available on IV therapy, Hormone optimization, and Weight loss packages
* Basic lab work one time per year (includes CBC, CMP, Lipid panel, Thyroid Panel, Vit D, A1C). Discounted lab fees available for any additional testing that may be needed.
Direct Primary Care (Small Businesses) - $69/month

Includes ALL the same services as DPC for individuals
Sick visit non- DPC Members - $69/visit
- Used for sick visits (in person or tele health)
- Additional fees may apply for any additional labs, procedures, or diagnostic test needed during visit

DPC is a membership-based healthcare model where patients pay a flat monthly fee for unlimited access to their provider without the hassle of insurance.
With DPC, there are no copays, no deductibles, and no surprise bills. You receive longer, more personalized visits, direct provider access, and a focus on preventative care.

Yes, we offer convenient labs on site at the practice. Every member gets their annual labs FREE!
If you need additional labs done, we offer the lowest cash-based pricing for labs, saving you valuable time and money on bloodwork.
DPC is NOT insurance! While you are not required to have an insurance plan with a Direct Primary Care membership, we highly recommend that you maintain medical insurance. Direct Primary Care can address up to 90% of your medical needs, but a DPC membership does not cover the cost of specialists, hospitalizations, emergency room and urgent care visits. Unexpected health emergencies, such as the need for surgery, can happen to anyone, and we recommend that you have coverage for these catastrophic events.

Yes, with a 30 day written notice, but we recommend a commitment of at least 3-6 months to experience the full benefits of DPC.
